Annual telehealth exam ($99 fee waived)

Cancellation Policy:

Member may cancel this membership after an initial 90-day period with a 30-day written notice to the Lounge. Cancellation requests must be submitted in writing at least 30 days prior to the desired cancellation date.

No cancellation fee will apply after the initial 90-day period.
Miscellaneous:

Agreement is non-transferable.

All services must be utilized while the membership is active.

The Lounge reserves the right to modify the terms and benefits of the membership with 30 days’ notice.
